Hot, cold, and when to use which

The instinct after any pain is to reach for heat, and sometimes that's wrong enough to slow recovery by a week. Cold is for fresh injuries and anything still swelling. It slows blood flow into the area, dulls the pain, and keeps inflammation from spreading, so it goes on in short windows right after the injury.

Heat is for older pain, the chronic stiffness, arthritis flare-ups, and muscle tension that have been hanging around. It opens blood flow and loosens the muscle so it actually moves, which makes it useful before activity rather than after. If a resident has both an old injury and a new flare, treat them as two problems with two approaches.

Match the brace to the stage, then the fit

A rigid brace or splint holds the joint still, which is what a fresh post-op or a bad sprain needs. A soft compression support lets the joint move while steadying it, which suits arthritis and everyday strain. The common mistake is leaving someone in a rigid brace past the rehab phase, so step the support down as the joint heals.

Fit is the other half. A brace sized too big slides and stops doing its job; too small and it digs in and gets left in a drawer. Going by the measurement instead of eyeballing it is what keeps the brace on the resident.
